What is electrical stimulation?
Electrical stimulation sends a gentle, controlled current into the muscle and nerve you are trying to reach. The current does three things at once. It activates muscle that is not recruiting on its own, it improves circulation through the area, and it retrains the nerve pathway that carries the signal. It is not a workout you have to perform. The current supplies the contraction, and your job is to feel it, follow it, and over time take it over yourself.
Where the current goes depends on what is failing. Your therapist may use an internal vaginal or rectal sensor to reach the pelvic floor directly, external TTNS placed on the leg to work on the nerve supply sitting behind bladder urgency, or surface placements on the abdomen or lower back. Who it's for.
Electrical stimulation suits muscle that will not switch on, and nerves that need reminding. Placement and intensity come out of your evaluation, not out of a standard protocol.
Leakage and urgency
Urinary or fecal leakage. The urge that arrives with no warning and rearranges the rest of your afternoon. Stimulation is used two ways here: directly at the pelvic floor to build a contraction you cannot yet produce, and at the leg with TTNS to work on the nerve supply that drives urgency. Which one you get comes out of the evaluation.
Weakness after pregnancy or surgery
Core and pelvic muscle weakness after birth, or after a pelvic or abdominal operation. The muscle is still there, but the connection to it has gone quiet, and you cannot contract what you cannot find. A current that produces the contraction for you gives the nervous system something to lock onto while strength gets rebuilt around it.
Pain and guarding that will not settle
Chronic pelvic, hip or lower back pain that has gone on long enough that you have stopped mentioning it to anyone. Here stimulation is used to improve circulation, ease discomfort, and quiet a guarding pattern that keeps reloading. Evaluation and settings

Placement and current

Afterwards
There is no downtime. You dress and go back to your day immediately, with no recovery period to plan around. Your therapist pairs the stimulation with hands-on work and a home program, then adjusts the settings as you progress. At the evaluation you get a realistic picture of what your own case looks like.
Electrical stimulation has real contraindications, so your therapist reviews your health history and precautions before the first pulse. Tell us if you are pregnant or think you may be, if you have a pacemaker, implanted defibrillator, neurostimulator or drug pump, an active malignancy in the treatment area, an active infection, an open wound or broken skin where electrodes would sit, a blood clotting disorder or a known clot, a metal implant in the treatment field, or reduced sensation in the area. Some of these change the plan. Some rule the modality out.
